Late Registration of Certificate of Death
OFFICE OF THE MUNICIPAL CIVIL REGISTRAR
This is the process of registering the Certificates of Death of those who died in Municipality in Buug after the thirty (30)-day filing period from the date of death.
Requirements

Certificate of Death (4 copies) Where to secure: Hospitals, Funeral Parlors, or other persons (Attendant)

Police Report (if applicable) Where to secure: PNP - Medico-Legal Section

Certificate of Burial/Cremation Where to secure: Cemetery/Columbarium
