Data Privacy Agreement

All Data and information (e.g., name, address, sex, age, and contact details) shared in this Client Satisfactory Measurement (CSM) shall be recorded and shall be treated with utmost confidentiality in accordance with the Republic Act No. 10173 or the Data Privacy Act of 2012. Thus, clients shall be asked to acknowledge and agree that their information may be used for the Municipal Government's documentation following applicable legal requirements on data protection. Any issue or concern on this matter shall be communicated to the Municipality of Buug for proper action.

Ang lahat ng impormasyon tulad ng pangalan, tirahan, at contact details, na ibinabahagi sa pamamagitan ng Complaint Form (CF) na ito ay itinatala at ginagamit nang may pag-iingat at naaayon sa Republic Act No. 10173 o "Data Privacy Act of 2012". Kung kaya, ang lahat ng kliyente ay iminumungkahi na bigyang-pahintulot ang Pamahalaang panglungsud na gamitin ang kanilang personal na impormasyon alinsunod sa Data Privacy Act. Anumang katanungan, suhestyon o alalahanin patungkol sa paunawang ito ay maaaring ipatuloy/idulog sa Tanggapan ng Punong Lalawigan o Local Govenremtn unit of Buug para sa karampatang aksyon.
